Can I choose my Physical Therapy Provider?

Yes, you have the right to choose your physical therapy provider. Your physician may suggest a facility, but you can choose whatever provider you like. Most people choose their provider based on a convenient location close to home or work, and one who is in the network with their health insurance plan. Do I need a prescription or referral from my Doctor?

What do I bring for my first visit?

For your first visit, you will need to bring your insurance card, a photo ID, and your prescription from your doctor (if applicable). When you call to schedule your appointment, you may also be asked to visit our website to print & fill out forms. Please fill out the requested forms and bring them with you on your first visit. It is best to wear loose, comfortable clothing and supportive shoe wear. If you are being treated for a hip, knee, or ankle injury, please wear or bring a pair of shorts.

What can I expect at my therapy appointment?

On your first visit, you will be greeted by our office manager and be asked to complete a few forms. Your physical or occupational therapist will provide an initial evaluation to determine your impairments and functional limitations, and develop a treatment plan designed to meet your needs and goals. Treatment begins on the first visit. Your treatment plan MAY include but is not limited to: therapeutic exercise, manual therapy, modalities such as heat or ice, a home exercise program. Visit lengths may vary, with most visits lasting approximately 1 hour. Treatment plans may consist of 1-3 visits per week. At OSPTA, we encourage treatment by the same therapist each visit for consistency of care.

Does my insurance cover physical therapy?

Please visit our insurance page to see if your specific plan is accepted. We accept most major insurance carriers. You will be given more specific information regarding what payment your insurance plan may require at your first visit.
